Fill out all sections except where signatures or notarizations are required
Bring a valid photo ID (see the accepted list below in Section 4)

🪪 What Types of ID Are Accepted for Notarization?
To complete any notarization, clients must present a valid, government-issued photo ID. Acceptable forms of identification follow the requirements of the state where the notarization takes place. Common examples include driver’s licenses, state ID cards, U.S. passports, military IDs, and other government-issued photo identification.
